5 Mistakes That Sink Radiology Center Startups

Most of the radiology center failures we have seen in our consulting work share the same five failure modes. None of them are about clinical quality, they are about financial planning, regulatory sequencing, and referral-network strategy.

  1. Underestimating RF and lead shielding costs. A 3T MRI room requires a Faraday cage (RF shielding) and adequate 5-Gauss line clearance from adjacent spaces. Shielding alone can cost $150,000-$400,000, a figure most early-stage capex models do not include. If you are retrofitting an existing commercial building, structural assessments for floor loading (a 3T magnet weighs 10-15 tonnes) add another $20,000-$60,000.   2. Opening without ACR accreditation. The ACR accreditation process takes 4-6 months. Many founders assume they will get accredited after opening and bill cash-pay patients in the interim.
